The Mulligan Concept

The Mulligan Concept, which Dr. Jacob Kurian of Enhanced Physical Therapy in Mineola uses, was initially created by Brian Mulligan, has over 100 articles regarding and supporting the concept. Brian Mulligan, a certified physical therapist from New Zealand, got his diploma in 1954. He has written two books, and his concept is world-renowned for it’s effectiveness.
The Mulligan Concept focuses on mobilization with movement. It also involves sustained natural apophyseal glides of the spine, also known as SNAGS. For these techniques to be used, there are some criteria a patient has to meet. There has to be either pain or loss of motion in the joints, or while performing a specific activity. While applying the Mulligan Concept, the therapist looks for something called the PILL response. The PILL response stands for Pain free, Instant result, and Long Lasting. If the PILL response is not observed, then the combination of gliding is not correct for that patient and a different method should be attempted. The second response principle is called CROCKS, and stands for Contra, or when the PILL response is not contraindication to the treatment, Repetitions, Over-Pressure, Communications, Knowledge, and the Sustainment of regulation.
As far as techniques go, there are several ways to go about performing the Mulligan Concept, for different regions of the body. Movement with Mobilization (MWM) is applied to all joints from fingers to toes. The SNAGS technique is used on the spinal joints, the rib cage and the sacroiliac joint. There are often used in weight bearing positions, and should result in pain-free actions. The NAGS therapy technique stands for Natural Apophyseal Glides, and encompasses treatment for the cervical and upper thoracic spine. This technique is very useful for limited spinal movement. Another technique is called Spinal Mobilization with Limb Movement, and is used to bring a limb through a range of motion that it wasn’t able to before.
