Sacroiliac (SI) Joint Pain Treatment in Mineola, NY
MULLIGAN CONCEPT
Sacroiliac (SI) Joint Pain
SI joint pain is felt low in the back, usually on one side, where the spine meets the pelvis, and can spread into the buttock. At Enhanced Physical Therapy in Mineola, we use Mulligan pelvic and hip mobilization with movement, belt support and strengthening to help you stand, walk and roll over in bed more comfortably.

Common signs of sacroiliac (SI) joint pain
- Pain on one side of the low back or buttock
- Pain standing on one leg, climbing stairs or getting out of a car
- Pain rolling over in bed
- Pain that may spread into the back of the thigh
- Pain after pregnancy or childbirth
What causes it
The SI joints pass load between the spine and the legs. Pregnancy and the months after childbirth, falls, uneven loading and weak hip muscles can all contribute.
How we examine it
Before any hands-on treatment, we ask about your health and screen for anything that needs a doctor first. We use a group of clinical tests to see whether the SI joint is a likely source, and check the low back and hip. We test Mulligan glides during the movements that hurt.
Mulligan techniques we use
- SI joint MWM. A gentle glide of the pelvis while you bend forward or back.
- Hip rotation MWM. A glide at the hip that can ease SI pain.
- Pelvic belt. A belt to support the pelvis during activity and exercise.
- Lumbar SNAGs. If the low back is also involved.
What the research says
Small randomized trials have found Mulligan techniques can reduce SI joint pain. A preliminary trial in women more than six months after a caesarean birth found adding SI joint mobilization to stabilization exercise reduced pain more than exercise alone. The research is early, so we combine it with strengthening.
What else is in your treatment plan
Your plan includes manual therapy, belt support where helpful, hip and core strengthening and advice on daily tasks and lifting children.
How long recovery takes
Many people improve over 4 to 8 weeks.
When to see a doctor first
Call your doctor, or get urgent care, if you have any of the following:
- Numbness in the saddle area or bladder or bowel changes: go to the emergency room
- Fever or unexplained weight loss
- Severe pain after a fall
- Morning stiffness lasting over an hour with other joints affected

Frequently asked questions
How do I know my pain is from the SI joint?
A set of clinical tests can point to it, along with where you feel the pain.
Can you treat SI pain after having a baby?
Yes. We see many new mothers. Pregnancy itself needs extra care and your doctor's approval.
Do SI belts help?
A pelvic belt can reduce pain during activity for some people.
Is SI joint pain the same as sciatica?
No, though SI pain can spread into the buttock and thigh. Our exam separates them.
